DENTAL CARE LOYALTY PLAN
INCLUDED ANNUALLY:
- 2 Oral Exams
- 1 Emergency Exam
- 2 Cleanings or Periodontal Maintenance
- 2 Oral Cancer Screenings
- 2 Fluoride tooth desensitizing treatments
- 1 Bitewing x-ray
- Any individual x-rays needed during the year
ADDITIONALLY RECEIVE:
- 25% off any additional preventative services
- 20% off UCR fees for basic service like fillings, extractions, root canals, etc.
- 15% off UCR fees for major services such as but not limited to implants, crowns, veneers, and more
Single: Individual Annual Plan
Dual: Plan for two individuals residing in the same household
Family Plan: 3+ Members, Add $200 per additional member after first two.
PROGRAM GUIDELINES:
- Program fee must be paid in full to be activated
- Plan fees are non-refundable even in the case of early cancellation
- All plan benefits must be utilized within 12-months of activation (benefit period).
- Plan automatically renews on anniversary date unless member provides notice to terminate 30-days prior to renewal date.
- A $50 reinstatement fee will apply if your plan lapses.
EXCLUSIONS & LIMITS:
- Discounts do not apply for Nitrous oxide, IV Sedation, medications, products, and/or other miscellaneous specialty services.
- Only dental procedures performed in this office are benefits under this plan.
- May not be used in conjunction with any other dental plan, dental insurance, or outside financing.
- Does not apply to the cost of care that is otherwise covered under an automobile or other injury liability coverage.
